Clinical Observation in Neonatal Sepsis


Abstract
Septicemia is still one of major cause of mortality and morbidity in neonatal period. And it is difficult to diagnose and to treat appropriately because of the clinical manifestations are subtle and rapid progress.
An analysis of 28 cases of neonatal septicemia confirmed by clinical manifestation and blood cultures from Jan. 1982 to Dec. 1991 at the nursery of Seoul Red Cross Hospital was done.
@ES The results were as follows:
@EN 1) The incidence was 2.4/1,000 live births. And the morbidity rate of prematurity(0.94%) was higher than that of full-term(0.18%) significantly(p<0.05).
2) According to the age at onset neonatal sepsis, the number of early-onset neonatal sepsis(12cases) were slightly less that of late-onset neonatal sepsis(16 cases).
3) The predisposing factors for neonatal sepsis were premature rupture of membrane, maternal amnionitis, prematurity/low birth weight infant, and perinatal asphyxia.
4) The associated focal infections in the neonatal sepsis were meningitis(25%), gastroenteritis (25%), pneumonia(14.3%), and etc.
5) The clinical manifestations observed in neonatal sepsis were temperature instability(75%). Jaundice(46.4%), respiratory symptoms(42.%), poor sucking(28.6%), irritability and etc.
6) The commonest organisms isolated from culture or latex agglutination showed Escherichia coli(6cases), Klebsiella pneumoniae(6 cases), Staphylococus epidermidis(6 cases), Staphylococcus aureus(4 cases), and Group B Streptococcus(3 cases).
7) The overall mortality rate of neonatal sepsis was 17.95.
